This PR updates Dynamo's vLLM backend to lower router-produced KV hints into vLLM's first-class KvHintsEnvelope request metadata. This keeps hint protocol data separate from connector-owned kv_transfer_params and aligns Dynamo with the API added by vLLM #53423.

Details

• Converts Dynamo's kv_hint request object into vLLM KvHintAction and KvHintsEnvelope objects.
• Passes the envelope through the dedicated engine_client.generate(..., kv_hints=...) argument for token, text, and prefill paths.
• Applies request hints once in P/D: the prefill worker receives the hint, while decode handoff requests with prefill-produced transfer parameters do not replay it.
• Restores kv_transfer_params to connector-owned metadata and preserves unrelated SamplingParams.extra_args through copy-on-write replacement.

KV hints and KV transfer parameters now use independent vLLM request surfaces:

Request lifecycle

_build_vllm_kv_hints() reads the router-produced envelope, constructs each typed action with its payload, and returns the canonical vLLM envelope. Requests without a hint continue through the existing path without passing a kv_hints argument.

For P/D, prefill passes the envelope to vLLM alongside fresh connector parameters. The decode request consumes the prefill-produced kv_transfer_params and omits kv_hints, preserving the existing apply-on-prefill behavior without nesting hint data inside connector metadata.
